We specialize in caring for Maine's aging adults with a whole-person approach that manages all your primary care needs in the comfort of your own home. Our Provider

Isaiah brings extensive experience from diverse healthcare settings, including serving as faculty at Maine Dartmouth Geriatric Medicine's Fellowship Program, providing veteran care through VA Home Based Primary Care, and working as Medical Director at Maine's largest private residential care organization. He earned his Bachelor's in Nursing from the University of Southern Maine and his Master of Nursing from Boston College, and is board certified in Adult & Gerontological Primary Care Advanced Practice Nursing.

As President of the New England Chapter of the Gerontological Advanced Practice Nurses Association and board member of Dirigo Geriatrics Society, he actively advances gerontologic nursing throughout the region. Isaiah founded Dome Health, LLC to improve care for individuals with complex health conditions while partnering with families, caregivers, and care organizations to deliver patient-centered care.

When not making house calls throughout Central Maine, Isaiah enjoys gardening and cooking at home with his life-partner Laura, their two sons, and two dogs.

Isaiah Meyer, MS, APRN, AGPCNP-C is a board-certified Adult Gerontology Primary Care Nurse Practitioner with over a decade of experience caring for older adults throughout Maine. He specializes in geriatric primary care, complex chronic conditions, dementia care, and innovative home-based healthcare delivery.

Our Staff

Laura Meyer brings a wealth of healthcare experience and deep community roots to her role as Practice Administrator at Dome Health, LLC, a family-run, mom and pop business she operates alongside her husband Isaiah. With a Master's degree in Traditional Chinese Medicine, she owned and operated a thriving acupuncture practice in Portland, Maine for eight years, developing extensive expertise in insurance billing, healthcare regulations including HIPAA compliance, and patient-centered care. Her diverse medical background includes working in medical records at Dominican Hospital, serving as a medical assistant at Santa Cruz Women's Health Center, supporting families as a labor and birth doula, and volunteering with the Santa Cruz AIDS Project.

Deeply woven into the fabric of Central Maine where she and Isaiah have raised their two children, Laura volunteers as a Master Gardener and helps refugees learn English. When she's not managing the practice, you'll find Laura tending her garden, caring for her chickens and dogs, and spending time with family. Her mission is simple but powerful: helping people feel their best so they can live their best life, and her combination of professional expertise and personal warmth makes her an invaluable partner in Dome Health's commitment to bringing quality geriatric care directly to the homes of rural Maine families

Home-based primary care brings your medical provider directly to you. We provide all typical primary care services plus specialized management of conditions that affect daily living, such as dementia and heart failure.
No, Dome Health will become your new primary care provider when you join our practice. This transition begins on the date of your first visit with us.
Yes. Once you complete your admission paperwork and are accepted as a new patient, we'll contact your previous provider and request your medical records.
No. All visits are delivered in-person where you live or via video calls. When you become a new patient, you'll receive detailed instructions about what to expect during your initial visit along with your new patient paperwork.
Yes! We provide care wherever you call home—including assisted living, memory care, and residential facilities. The only exception is nursing homes, where you're required to see the on-staff medical provider and cannot choose an outside primary care service.
We're here when you need us! Dome Health is available 24/7 through our ongoing care, acute services, and on-call support. For stable health conditions, we typically recommend visits every 3 months to manage chronic conditions and include routine visits like annual check-ups. However, your care plan is personalized based on your individual health needs.
Absolutely! Video visits help provide convenient access to your healthcare provider. Many check-ins and non-urgent needs can be handled via video through your computer, smartphone, or tablet. Visit the patient portal for setup instructions.

Business hours: 8:00 AM to 4:30 PM EST Mon-Fri. For medical emergencies call 911. For other urgent matters, we have a provider on call 24 hours a day 7 days a week. Call our office and follow the instructions to reach our on-call provider.
We accept Medicare, MaineCare, and most commercial insurance plans.
Billing works similarly to your previous primary care provider. Your insurance will be billed directly for services. If you have a co-payment or co-insurance, you'll be notified and can pay at the time of your visit. For specific questions about coverage, contact your insurance provider and ask about home visit benefits.
Many of our patients have designated healthcare decision-makers due to cognitive or functional needs. These may include: Healthcare proxy or surrogate decision maker, Power of Attorney for healthcare, or Legal guardian. Please provide documentation of any designated decision-makers with your admission paperwork. This ensures they have access to important information and can fully support your care.
